The Ambulatory Clinical Specialist Pharmacist will provide expert, patient-centered pharmaceutical care in a clinic-based ambulatory setting. The ambulatory specialist will monitor drug therapy for patients, assessing therapeutic endpoints and potential adverse effects through evaluation of relevant laboratory and diagnostic parameters. They will provide therapeutic drug monitoring and make recommendations to optimize outcomes and reduce the risk of adverse effects. The specialist will participate in clinical services in accordance with applicable policies, procedures, collaborative practice agreements, and protocols. Functioning effectively as part of the interdisciplinary team, the pharmacist will serve as a clinical resource to healthcare providers, collaborate across teams to support evidence-based care, and contribute to the advancement of the pharmacy department’s professional and clinical services through program development, education, and quality improvement initiatives.

Degrees:

• Doctorate.

Licenses & Certifications:

• Basic Life Support.
• Registered Pharmacist.

Additional Qualifications:

• Doctorate of Pharmacy with Board Certification of Pharmacy Specialty (BPS) required in area of specialty practice.
• If not Board Certified by BPS in area of hire, Pharmacy Practice Residency PGY2 and achievement of Board Certification within 1 year of hire is required.
• If no residency training, must have > 5 years of practice experience in area of specialty and Board Certification in area of specialty practice (PGY1 = 3 years of experience & PGY2 = 2 additional years).
• Florida Consultant Licensure within 3 months of hire.
• Immunization certificate within 6 months of hire.

Knowledge of Microsoft office products, including Word, Excel, PowerPoint and other graphics or presentation software
