Clinical Pharmacy Specialist - Ambulatory/Primary Care (Medford, OR)
The Clinical Pharmacy Specialist (CPS) will collaborate with primary care physicians and staff to provide coordinated, comprehensive, high-quality care to PMG patients. The responsibilities of the Clinical Pharmacy Specialist predominantly fall into the following areas: (1) direct patient care, (2) population-based disease management, (3) preceptor/educator, (4) pharmacy support, (5) projects.  (1) Patient Care - The Clinical Pharmacy Specialist is responsible for providing comprehensive medication management services in designated PMG clinics in-person, telephonically, and by e-visits. Upon referral or identification, the Clinical Pharmacy Specialist will assess the patient, adjust medications, screen for drug interactions and adverse effects, provide disease/medication educatio
