As a Clinical Pharmacy Specialist:
Provides a minimum of two (2) in-services per year on rational medication therapy and medication procedures effectively to health care professionals and patients.
Actively participates in the provision of training and mentorship within peer group and cross campus pharmacists in a particular service line including pharmaceutical care and disease state management.
Works closely with multidisciplinary team for an assigned service line and provides backup support to service lines where appropriate and as assigned.
Responds to cardiopulmonary arrest scenarios for service line and backup for other units when needed.
Maintains active physical presence on patient care units for 80% of shift with consideration for lunch, meetings, presentations, interviews, project days, and precepting.
Participates as an active committee member within assigned service line and additional meetings as assigned.
Provides clinical expertise in area of service line as well as additional clinical programs as assigned.
Serves as primary preceptor for pharmacy students and residents in a particular service and fulfills all documentation responsibilities.
Communicates effectively with healthcare professionals, patients and family members where appropriate.
Actively participates as a member of the local, state, and national pharmacy societies.
Creates national visibility by presenting clinical information/programs such as poster presentations or managed case studies or publishing in nationally recognized journals.
Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, disease state appropriateness relative to drug and dose.
Actively participates across the campus structure aiding in recommendations for appropriate, cost-effective therapeutic medication alternatives to medical staff and peers.
Actively participates in stewardship activities and restricted mediation programs.
Provides accurate and timely medication information as needed to all healthcare professionals.
Takes a lead role in working within clinical initiatives, improving patient safety and medication use.
Facilitates or assists in the medication utilization evaluation process for service line.
Documents all clinical activities and interventions accurately and completely
Supports and participates in the goals of the organization's quality improvement, risk management, and patient safety programs to include, but limited to, National Patient Safety Goals, Core Measures, regulatory compliance and HCAHPS medication-related initiatives
Prioritizes work and assists in implementation of clinical process changes and others as assigned.
Participates in all aspects of managing the drug formulary.
Participates in additional department functions to support the operations of the pharmacy department.
Competent to fulfill Pharmacist job responsibilities when needed.
Internal Medicine Specialist - practice out of Chippenham Medical Center - rotates between Med/Surg units decentralized during day shifts, every 4th weekend central staffing rotation, 1 minor holiday, 2 day major holiday.
Heme/Onc Specialist- practice out of Johnston Willis Hospital - staffs Oncology inpatient unit and OPIC oncology order verification, every 4th weekend central staffing rotation, 1 minor holiday, 2 day major holiday.
Clinical Pharmacist - Mixed Rotating Shifts - available at Chippenham or Johnston Willis - rotates between trained units (including med/surg, trauma, ED, ICU, central staffing) - every 3rd weekend central staffing rotation, 1 minor holiday, 2 day major holiday.
EXPERIENCE
Required - Effective communication skills are required. Effective written and verbal skills are required.
Required - Experience using computer applications including word processing, spreadsheet software and pharmacy information systems.
Required – Interest/knowledge in pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics in specialty service
Internal Medicine Clinical Specialist @ Chippenham - Required:
Minimum 1 year’ hospital experience in specialty area OR minimum 2-year hospital pharmacy experience PLUS one of the following:
Completion PGYI residency program
Completion of PGY2 residency program
Board Certification (general or in specialty area (preferred))
Oncology Clinical Specialist @ Johnston Willis -Required: Over 5 years’ experience in specialty area OR minimum 2-year hospital pharmacy experience PLUS one of the following:
Completion PGYI residency program
Completion of PGY2 residency program
Board Certification (general or in specialty area (preferred))
EDUCATION
Required - Graduate of an ACPE-accredited school of pharmacy at a college or university.
Required - Completion of two years of hospital pharmacy experience or an ASHP - approved pharmacy residency program.
Licensure/Certification
Required - Licensure by the Virginia State Board of Pharmacy by examination or reciprocity.
Required – BLS & ACLS (IF pediatric clinical pharmacy specialist, PALS will be substitute for ACLS).
Preferred: Doctor of Pharmacy from an ACPE accredited college of pharmacy
Preferred: Completion of PGY2 residency program in specialty area
