The University of Toledo College of Pharmacy and Pharmaceutical Sciences Department of Pharmacy Practice and UToledo Health is seeking two motivated pharmacists to fill two shared faculty positions, within ambulatory care practices. Each position will be split 60% clinic and 40% teaching.
Duties: Overall duties include didactic and experiential teaching, patient care, scholarly activity, and service. The individual will teach in the PharmD program, and will develop and maintain an interprofessional collaborative practice with a UToledo Health clinic. The academic responsibilities of the clinical faculty member may include teaching in the classroom and skills-based laboratory portions of the didactic curriculum, precepting PharmD students (IPPEs and APPEs) and PGY1 residents, and advising students.
Scholarly activity, including coordinating and conducting quality improvement projects, developing collaborative treatment and research protocols, and presenting and publishing original work is encouraged and will be supported.
Academic rank appointment will be commensurate with experience.
Primary care clinic
This clinical practice includes a group of well-established, clinical pharmacists embedded in a robust collaborative practice with physicians, APPs nurses, and other healthcare professionals. The clinical practice will focus on providing care under a collaborative practice agreement for disease states such as diabetes, hypertension, dyslipidemia, and tobacco cessation and to develop and implement population health management based quality initiatives.
Psychiatry clinic
The clinical practice will include the creation of an embedded clinical pharmacist with a focus on collaborating with physicians, mid level providers, and clinic teams to promote best evidence-based care with our clinical staff to ensure access to therapies and excellent patient experience as it relates to psychiatric therapeutics. Pharmacists will conduct patient assessments and education as well as managing clinical follow ups and interventions to optimize patient outcomes.
