Pennsylvania Hospital is seeking a full-time, day shift Hematology/Oncology Clinical Pharmacy Specialist. This position is responsible for provision of comprehensive medication management to patient populations across the institution including ambulatory and acute care. This pharmacist serves as a resource to Pennsylvania Hospital on best practices, evidence-based guidelines, emerging therapies, transitions of care, optimal pharmacologic management, effective medication reconciliation, and other pharmaceutical services.
