Purpose:

PGY2 residency programs build upon Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency training to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives for advanced practice areas. Residents who successfully complete PGY2 residency programs are prepared for advanced patient care or other specialized positions, and board certification in the advanced practice area, if available.

Overview:

The University at Buffalo School of Pharmacy and Pharmaceutical Sciences (UB SPPS) and Buffalo Medical Group (BMG) offer a PGY2 Ambulatory Care Residency with 2 open positions. Strengths of this program include Patient Care, Population Health, Teaching and Research.  The resident’s disease state management training will include diabetes management, heart disease risk reduction, CMR, MTM and medication reconciliation. The resident will be involved in the provision of patient care in-person and through telemedicine technology.

The resident will provide Population Health Management related to HEDIS measures, STAR ratings, and other quality improvement initiatives. Patient Care occurs in a longitudinal manner over the course of the year to provide the resident with ample opportunity to build relationships and follow patients over time. The resident participates in many aspects of pharmacy including drug information, research, patient-centered care with interdisciplinary teams, PCMH management, and population health initiatives.

Candidates must possess a PharmD degree from an ACPE-accredited school of pharmacy, have completed an ASHP-accredited PGY1 Pharmacy, Community Pharmacy or Managed Care Pharmacy residency and be eligible for licensure in New York State. In addition, the incoming resident must have or obtain Immunization Training and Certification.
