One Brooklyn Health comprises of three main campuses: Brookdale Hospital Medical Center (BHMC), Interfaith Medical Center (IMC) and Kingsbrook Jewish Medical Center (KJMC). As a system, OBH is transforming healthcare delivery for Brooklyn's most vulnerable residents through the development of a regional ambulatory care network and the integration of key clinical services. In addition, OBH has two large nursing homes within its system, Schulman S. Schachne Institute for Nursing and Rehabilitation at BHMC and Rutland Nursing Home at KJMC.
BHMC is one of the most established and largest teaching institutions serving the Brooklyn area. The institution serves as a Level I Trauma Center and operates over 500 licensed beds. The Department of Pharmacy provides a myriad of services which include centralized and decentralized satellite pharmacies, a Clinical Pharmacy Program, collaborative practice clinics, and transitions of care services including a meds-to-beds program. The Clinical Pharmacy Program includes pharmacists in the areas of Ambulatory Care, Anticoagulation/Cardiology, Critical Care, Emergency Medicine, Infectious Diseases, Internal Medicine, and Psychiatry. The Department of Pharmacy also has affiliations with Long Island University and Touro Colleges of Pharmacy
Program Purpose
The PGY-2 Ambulatory Care Residency program builds on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in Ambulatory Care. Our program provides residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification.
Program Description
The One Brooklyn Health's PGY2 Ambulatory Care Residency is an organized, directed, postgraduate training program in ambulatory care pharmacy practice. The goal of this residency program is to enable residents to develop the knowledge and experience necessary to provide pharmaceutical care to ambulatory patients with a strong emphasis on developing and mastering clinical skills in the outpatient setting.
This program will offer the resident the opportunity for longitudinal patient care experiences within our collaborative practice clinics the Medication Management (primary care, diabetes, heart failure, hypertension, asthma, and chronic obstructive pulmonary disease), Anticoagulation (point-of-care testing) , and Special Treatment and Research (STAR) HIV Clinics. Additional longitudinal experiences include the Psychiatry Clinic, Transitions of Care, Community Outreach, Ambulatory Practice Management, and Research. Primary responsibilities of the resident will include conducting direct patient visits, managing clinic operations, maintaining and reporting of clinic interventions, and collaborating with the interdisciplinary ambulatory team.
Teaching experience is also gained by completing monthly lectures during Pharmacy Grand Rounds for pharmacists, pharmacy residents, pharmacy students, and pharmacy technicians. Residents will also have the opportunity to serve as a co-preceptor for pharmacy students and mentor PGY-1 Pharmacy residents.
Program Structure
The residency is conducted primarily as longitudinal clinic experience carried out at various ambulatory care clinics. These clinics include the Medication Management, Anticoagulation, Psychiatry and HIV Clinics. The resident is simultaneously expected to prioritize and work on the ongoing clinical, administrative, didactic, research and project activities. There is the opportunity for elective rotations that can be completed at the institution to provide a well-rounded experience. The resident works closely with the residency program director and primary preceptors to develop goals and objectives that will assist the resident in expanding skills for current and future practice. Regular meetings with the residency program director ensure that long-term goals of the residency program are fulfilled.
