The Clinical Pharmacist (Inpatient) is located within the Pharmacy Service of the Kansas City VA Medical Center (KCVAMC). The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Major Duties include but are not limited to the following: Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and the formulary issues. Coordinates workload activities and expected to direct colleagues, technicians, and subordinates to complete routine tasks of the shift and assigned area. Responsible for developing and maintaining evidence-based treatment protocols and assisting with criteria for use and new medication requests, for their service. Identifies and prioritizes the patient's pharmaceutical problems and initiates and executes evidence-based pharmacotherapeutic regimens and corresponding monitoring plans. Reviews and interprets appropriate laboratory tests and other studies necessary to perform and interpret the physical measurements necessary to monitor, support and modify the patient's medication therapy. Monitors and assesses patient's outcomes to drug therapy. Detects, reports, monitors, and manages adverse drug reactions. Intervenes as necessary to increase effectiveness and minimize risks of drug therapy and improve health outcomes. Effectively monitors and assesses patient's outcomes to drug therapy. Consistently detects, documents, reports, monitors, and manages adverse drug reactions. Intervenes as necessary to increase effectiveness and minimize risks of drug therapy and improve health outcomes. Formulates an efficient and thorough approach to retrieving and analyzing drug information and provides professional and timely written/oral response to the patient or appropriate member of the health care team. Writes professional and concise entries in the medical record. Completes all documentation accurately and in a timely manner allowing for optimal workflow and appropriate and efficient patient care. Evaluates procedures/process and demonstrates the ability to make timely and effective decisions that facilitate change and produce results that improve performance or patient care. Assesses the educational needs of medical center staff and provides appropriate and timely formal/informal education on an ongoing basis. Designs and coordinates drug utilization evaluations and the clinic's quality assurance program. This includes the collection and compilation of data and the preparation of reports to be provided to the appropriate committees. Serves as a research advisor to pharmacy residents for their residency project. Other related duties as assigned. Work Schedule: Monday Thru Friday dayshift. Will assist with off tours per manager.
