This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The Pain Clinical Pharmacist Specialist (CPS) must be a licensed pharmacist with a Doctor of Pharmacy Degree and completed an American Society of Health System Pharmacists Accredited Clinical Pharmacy Practice Residency or other equivalent experience. The Pain CPS is responsible for providing evidence-based pharmaceutical care services and supporting rational cost effective drug therapy to improve patient outcomes in Pain Management. Duties related to this position includes but not limited to: Serves as an expert for the pain program, evaluation, implementation and maintenance. Evaluates a wide variety of patients in assigned patient care areas and makes appropriate recommendations on pain therapy. Designs and recommends pharmacotherapeutic regimens and corresponding monitoring plans. Monitors and analyzes drug therapy regimens with health care providers (based on pharmacokinetics, patient disease states, and medication history) to provide the most effective, least toxic, economical and risk mitigated drug treatment plan. Makes recommendations to optimize drug therapy, prevent and manage adverse drug events, and reduce medication errors. Interviews patients (via in-person, telephone or telemedicine) to determine medical history and reviews history for potential drug interactions, poly-pharmacy, therapeutic duplication, noncompliance, dosing errors and adverse drug reactions. Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.
