The Anticoagulation Clinical Pharmacy Specialist (CPS) is responsible for all aspects of pharmaceutical care for Veterans in the anticoagulation clinic at the Minneapolis VA. The CPS is a licensed pharmacist that has knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics for anticoagulation therapy. Major Duties: The anticoagulation CPS provides appropriate selection of anticoagulant therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing. Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. Assesses drug safety and efficacy, including evaluation of physical symptoms. Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program. Recommends formulary alternatives and completes non-formulary consults as assigned. Responsible for pharmacy activities involving highly innovative methods of health care delivery in clinic and through telehealth or phone visits. Serves as a credentialed, mid-level provider by directing therapy; ordering lab tests; performing physical assessments; and initiating, modifying and discontinuing medications as defined within the Clinical Pharmacy Specialist's scope of practice (SOP).The CPS will be involved in the independent professional practice of pharmacy in accordance with the guidelines set forth in pharmacy service policies and procedures, VA regulations, and state and federal laws governing the practice of pharmacy. Maintains work flow patterns that assure timely delivery of anticoagulation services, ensures accurate and timely documentation of encounters and follow up orders. Assists in the training of pharmacy employees, pharmacy residents and Pharm.D. students as assigned.
