The Multiple Myeloma clinical pharmacy specialist ­­­­is responsible for providing clinical pharmacy services to patients in the Multiple Myeloma disease area. The pharmacist will work collaboratively with other disciplines to optimize the pharmaceutical care of these patients, including medication reconciliation, patient education, drug therapy monitoring, supportive care management, and medication/chemotherapy review. The pharmacist will also participate in the development of policies, procedures, teaching documents, education and/or monitoring tools, workflow design, and other initiatives that will improve and/or expand the program.

Pharmacists are responsible for the optimization of drug therapy by identifying, resolving, and preventing drug-related problems and adverse drug events.

• Evaluates medication appropriateness
• Provides drug information, patient education and clinical services
• Ensures the daily operations of the multiple myeloma clinical pharmacy services
• Participates in process and quality improvement
• Has a working knowledge of and is able to use all technology required to perform daily duties.

• Provides education to nursing, pharmacy and medical staff.

MINIMUM JOB QUALIFICATIONS:

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy and must be licensed to practice pharmacy in the Commonwealth of Massachusetts. 
• 3 to 5 years of hospital pharmacy experience is required as well as a minimum of 4 years of oncology speciality experience or the completion of a PGY-2 Oncology residency.
