The Montefiore Einstein Center for Transplantation is one of the most active and successful programs in the country, performing adult and pediatric heart, lung, kidney, liver, and pancreas transplants. Our approach unites the outstanding clinical staff at Montefiore Medical Center with the world-renowned basic science faculty at Albert Einstein College of Medicine. This enables us to offer innovative treatments and access to advanced clinical research trials.
The transplant clinical pharmacist holds an integral role within the inpatient and outpatient transplant multidisciplinary teams. This challenging role calls for the clinician to be responsible for effectively enhancing the drug therapy outcomes of transplant recipients at the medical center. This is accomplished through continuous assessment of the appropriateness, safety and efficacy of individualized transplant patient drug therapy in the pre- and post-transplant period. The pharmacy clinician provides recommendations, drug information and education to the medical, surgical and nursing staff, pharmacy personnel, and other health care professionals. Additionally, clinical pharmacists actively participate in clinical research in transplantation or other related disciplines. The practitioner is eligible for faculty appointment at the Albert Einstein College of Medicine and the University at Buffalo School of Pharmacy and Pharmaceutical Sciences.
The Solid Organ Transplant Clinical Pharmacy Manager must have advanced training, including completion of a PGY-2 residency in solid organ transplantation or completion of a PGY1 residency with a minimum 3 years experience in this area of specialty. Must also be eligible for both board certification in solid organ transplantation and licensure in the state of New York. Board certification is supported by the Montefiore Department of Pharmacy.
