Located in the heart of Phoenix, Arizona, St. Joseph’s Hospital and Medical Center (SJHMC) is a 604-bed, not-for-profit academic teaching hospital. St. Joseph’s is a nationally recognized center for quality quaternary care, medical education, and research. SJHMC is home to the internationally renowned Barrow Neurological Institute, Norton Thoracic Institute, and Dignity Health Cancer Institute at St. Joseph’s Hospital and Medical Center. SJHMC also offers a Level III Neonatal Intensive Care Unit and a Level I Trauma Center certified by the American College of Surgeons. Residency Program Specifics The twelve month program provides the training necessary for graduates to provide exceptional pharmaceutical care to diverse patient populations. Additionally, the program offers the flexibility to tailor resident experiences in order to meet specific professional goals. Required Experiences* · Internal Medicine Four Elective Experiences from the Following* · Solid Organ Transplant Residents also complete a teaching certificate and help precept pharmacy students from the University of Arizona, Midwestern University, and Creighton University. Application Information Applicants must have a PharmD Degree from an ACPE-accredited school of pharmacy prior to starting the program. Applicants must also be a licensed pharmacist or eligible for licensure in the state of Arizona. The completed application packet must be submitted to PhORCAS by January 3, 2021. The application packet must include: 1) PhORCAS application form 2) Letter of intent 3) Curriculum Vitae 4) Official transcripts of all pharmacy education 5) Three letters of recommendation (preferably at least two from clinical APPE preceptors) ASHP Match Program Codes:
