At Stony Brook Medicine, a Transplant Coordinator is a valuable member of our team, who provides clinical services to our patient population.

Duties may include the following but are not limited to:
Working with pre-transplant multi-disciplinary team to conduct new patient evaluations and re-evaluations of patients on annual basis.
Coordinates patient entry and introduction to the transplant program.
Coordinates pre-transplant education for patients and their caregivers
Works with the outreach coordinator to provide professional education for community nephrologists and dialysis center staff
Responsible for outreach activities and events for patients and referring providers
Will act as the liason between the transplant center and the community dialysis centers and Nephrology offices
Responsible for obtaining patient referrals
Responsible for providing education to patients and providers both on site and out in the field
Coordinates medical, surgical, financial, psychosocial, nutrition, and pharmaceutical evaluation and on-going care needs of pre transplant patients.
Participates in patient care planning and selection conference.
Lists patients for transplant in a timely manner to facilitate access to donated organs.
Helps to maintain the UNOS waiting list according to program policies and procedures, regulatory requirements, and national guidelines.
Remains knowledgeable of all listed patients' status to ensure patient safety and facilitate organ placement in collaboration with transplant physicians.
Works with multidisciplinary team on performance improvement projects
Attending meetings, national, regional, local and internal.
NYS licensed RN
Knowledge of Office/EXCEL.
Excellent communication skills.
CCTC (Certified Clinical Transplant Coordinator).
Experience with ESRD/kidney transplant.
